YEAR-ROUND INSURANCE ELIGIBILITY STATED ON EQUITY INSURANCE PREMIUMS TAX
RECEIPT

Valuable improvements are included in the new Equity member insurance
plan debuting April 1, 2013.

An important new benefit is year-round coverage for sickness. An Equity
member who was insured for a minimum of eight (8) weeks in the previous
calendar year is now eligible for benefits 365 days a year. Previously,
only expenses incurred as a result of an accident were covered while a
member was not under contract. Under the new plan, 50% of the cost of
drugs and paramedical services required because of accident or sickness
will be reimbursed, subject to annual limits.

Eligibility for year-round coverage in 2013 was determined by the number
of days/weeks an Equity member worked in 2012. Going forward,
eligibility will be re-assessed annually based on work weeks at Premium
Levels 2 to 4. 

Insurance premium tax receipts for 2012 were mailed to the membership
earlier this week. Each tax receipt also identifies a member's
eligibility for year-round sickness coverage in 2013. 

Equity members should review their tax receipt's insurance eligibility
statement to determine if they qualify for year-round, off-contract
sickness benefits.
