CHUBB NOW ACCEPTS INSURANCE CLAIMS BY EMAIL

Equity is pleased to announce a new member service intended to speed up
insurance claim response times. Beginning April 21, Equity's insurance
provider will accept member claim submissions by email.‎

The new email address for claims and information is: CAEA.Claims at Chubb.com

Submitting a claim is now as easy as:
* SAVE your completed fillable insurance form
   * SCAN your receipts
     * EMAIL your forms and receipts to CAEA.Claims at Chubb.com
       * RETAIN your receipts‎

SAVE: Make sure to save the completed fillable insurance form to your
files. The online forms provide for the use of a digital or image
signature. Open the "Fill & Sign > Place Signature" feature in Adobe
Reader for more information. Members may also email an insurance form
without a signature provided it is sent from the member's email address on
file with Equity.

SCAN: Include all necessary receipts and required documents.

EMAIL: Review the claim form checklist before emailing your claim to avoid
any unnecessary processing delays‎.

RETAIN: Keep all original receipts for one year.‎
