<div><font size="3" color="#262626"><span style="font-size:12pt;">Through this RFO, the City will be re<font color="#1F497D">-</font>allocating the funds that are currently invested in Housing-based Case Management (<font color="black">HBCM). No additional
funding is available.</font> The goals of HBCM <b>are to support people who are experiencing significant periods of homelessness </b>to move into appropriate housing, learn relevant life skills, access community supports, achieve successful tenancies, <font color="black">and
work toward recovery and well-being.</font><font color="#1F497D"> </font></span></font></div>

<div><font size="3" color="#262626"><span style="font-size:12pt;">Agencies may apply to provide HBCM <font color="black">services</font><font color="#1F497D"> </font>for any of the following clienteles:</span></font></div>

ouvert et transparent pendant la transition de l’ancienne Stratégie des partenariats de lutte contre l’itinérance au nouveau programme Vers un chez-soi.</span></font></div>

<div><font size="3" color="#262626"><span style="font-size:12pt;">Dans le cadre de cette demande d’offres, la Ville réaffectera les fonds actuellement investis dans la Gestion de cas en matière de logement (GCML), mais elle ne dispose pas d'autres fonds pour
l'instant. La GCML a pour objectif d’<b>aider les personnes qui connaissent de longues périodes d’itinérance </b>à emménager dans un logement adéquat, à acquérir des compétences de vie pertinentes, à avoir accès à des mécanismes de soutien communautaire, à
devenir de bons locataires et à travailler à leur rétablissement et à leur bien-être. <b> </b></span></font></div>
