Le Forum a réuni environ 85 représentants d’entités communautaires (EC) de partout au pays. Les principaux objectifs du Forum étaient les suivants :

<p class="MsoNormal"><span lang="FR-CA">L’Alliance canadienne pour mettre fin à l’itinérance (ACMFI), en collaboration avec Iain, a animé des séances en petits groupes qui ont permis aux participants de partager leurs expériences et d’échanger leurs connaissances
sur la mise en œuvre de Vers un chez‑soi. Le partage des connaissances et la mobilisation à l’échelle communautaire ont été renforcés par trois groupes de discussion distincts où les EC de communautés de partout au Canada ont partagé leurs points de vue sur
le rôle du leadership dans la prévention et la réduction de l’itinérance.</span><span lang="FR-CA"><o:p></o:p></span></p>

<p class="MsoNormal"><span lang="FR-CA">Ce mois‑ci, la chronique <i>Pleins feux sur la communauté
</i>met en vedette Collingwood, en Ontario. Aux prises avec un nombre appréciable de jeunes en situation d’itinérance et une offre réduite de ressources en matière de logement, la communauté de Collingwood, en Ontario, a envisagé des approches préventives à
faible coût. Dwayne O’Connor, directeur général de Home Horizon, explique :<o:p></o:p></span></p>
<p class="MsoNormal"><i><span lang="FR-CA">Collingwood a un important problème d’itinérance chez les jeunes. Parallèlement, il y a très peu de ressources physiques en logement dans la communauté. La Ville de Collingwood et ses partenaires locaux, y compris
la bibliothèque de Collingwood, le Centre jeunesse de Collingwood et le Programme d’appui transitoire Home Horizon, ont élaboré des réponses pour lutter contre l’itinérance chez les jeunes. Ces mesures sont peu coûteuses, elles peuvent être immédiatement mises
en œuvre et sont préventives plutôt que réactives, et elles ne nécessitent pas la construction de refuges d’urgence ou de transition.

<p class="MsoNormal"><i><span lang="FR-CA">L’une de ces initiatives est la réconciliation familiale, qui empêche les jeunes de vivre l’itinérance en les aidant à renouer avec leur famille. L’autre initiative est HomeShare, qui jumelle des jeunes en situation
d’itinérance compatibles avec des membres de la communauté qui ont de l’espace disponible chez eux. Depuis l’ouverture de Home Horizon en septembre 2017, 17 jeunes ont renoué en toute sécurité avec leur famille. HomeShare, un nouveau programme lancé cette
année, a permis de réaliser un jumelage à ce jour. </span></i><i><span lang="FR-CA"><o:p></o:p></span></i></p>

<p class="MsoNormal" style="margin-bottom:0cm;margin-bottom:.0001pt"><span lang="FR-CA">La fonction Réservations dans les refuges du SISA permet aux fournisseurs de services de travailler ensemble pour gérer plus efficacement les admissions dans les refuges
dans un système de soins et de planifier la nuit à venir lorsqu’ils s’attendent à ce qu’un client revienne pour obtenir des services. Cela permet d’améliorer la prestation des services en créant des transitions plus homogènes entre les fournisseurs ou entre
les quarts de travail au sein d’un même refuge, ce qui réduit les traumatismes que peuvent subir les clients en minimisant le besoin de répéter l’information durant les processus d’admission répétés.

<p class="MsoNormal" style="margin-bottom:0cm;margin-bottom:.0001pt"><span lang="FR-CA">Les travailleurs de première ligne peuvent utiliser la fonction Réservations dans les refuges du module Admissions pour réserver des lits dans les refuges pour les clients
avant leur arrivée. Par exemple, les travailleurs d’approche peuvent aiguiller les clients vers un fournisseur de refuge particulier. Le fait de réserver un lit dans ces situations aide les fournisseurs de refuges à accélérer le processus de réservation lorsque
le client se présente pour obtenir des services. Il n’est pas nécessaire de répéter les détails partagés pendant la conversation sur la prévention et la déjudiciarisation, ce qui permet d’offrir un service plus homogène.

place pour les personnes qui ont besoin d’aide pour la désintoxication. Un client peut recevoir un traitement dans un centre de désintoxication et avoir besoin d’un lit à un certain moment, que ce soit le jour même ou dans un mois.<o:p></o:p></span></p>

<p class="MsoNormal">Iain DeJong, President and CEO of OrgCode Consulting and a recognized global leader in ending homelessness, led leadership-focused sessions on vision-setting and buy-in, leading community-wide efforts to prevent and reduce homelessness,
and aligning community interests by developing partnerships and taking action. <o:p>
</o:p></p>
<p class="MsoNormal">The Canadian Alliance to End Homelessness (CAEH), in collaboration with Iain, facilitated breakout sessions, which provided an opportunity for participants to share experiences and exchange knowledge on Reaching Home implementation. Community-level
knowledge sharing and engagement were further supported through three separate panels where CEs from a communities across Canada shared their insights on the role that leadership plays in the work to prevent and reduce homelessness.<o:p></o:p></p>
<p class="MsoNormal">Employment and Social Development Canada (ESDC) would like to thank of all the participants and presenters at the Forum. The feedback provided by participants over the two-day Forum will be used to improve future engagement activities and
inform efforts to assist communities in the implementation of Reaching Home.<o:p></o:p></p>

<h2><a name="_Article_Title"></a><a name="_Youth_Homelessness_Prevention:"></a>Youth Homelessness Prevention: Collingwood, Ontario<o:p></o:p></h2>
<p class="MsoNormal">This month’s Community Spotlight is on Collingwood, Ontario. Faced with a significant number of homeless youth and a small supply of housing resources, the community of Collingwood, Ontario looked to low-cost, preventative approaches. Dwayne
O’Connor, Executive Director of Home Horizon, tells us:<o:p></o:p></p>
<p class="MsoNormal"><i>Collingwood has a significant youth homelessness issue. At the same time, there are very few physical housing resources in the community. The Town of Collingwood and local partners, including the Collingwood Library, Collingwood Youth
Centre and Home Horizon Transitional Support Program, developed responses to combat homelessness among youth. These measures are low-cost, immediately implementable and preventative rather than reactionary, and do not require the construction of emergency
or transitional shelters. <o:p></o:p></i></p>
<p class="MsoNormal"><i>One of these initiatives is Family Reconciliation, which prevents youth from experiencing homelessness by reconnecting them with their families. The second is HomeShare, which pairs compatible homeless youth with community members who
have available space within their homes. Since Home Horizon opened in September 2017, 17 youth safely reunited with their families. HomeShare, a new program this year, has had one successful pairing to date.
<o:p></o:p></i></p>

<h2><a name="_Tips_&_Tricks:"></a>Tips & Tricks: Maximize Efficiency Using the Shelter Reservations Feature<o:p></o:p></h2>
<p class="MsoNormal">The shelter Reservations feature in HIFIS gives service providers the ability to work together to manage shelter book-ins more efficiently in a system of care as well as plan for the night ahead where they expect a client to return for
service. This allows for better service delivery by creating more seamless transitions between providers or between shifts within the same shelter, both reducing the trauma that clients can experience if they have to repeat information during repeat intake
processes. <o:p></o:p></p>
<p class="MsoNormal"><b>Reservations<o:p></o:p></b></p>
<p class="MsoNormal">Front line workers can use the shelter Reservations feature in the Admissions module to reserve shelter beds for clients before their arrival. For example, outreach workers can refer clients to a specific shelter provider. Reserving a bed
in these situations helps shelter providers fast-track their book-ins when the client shows up for service. Details shared during the prevention and diversion conversation do not have to be repeated, allowing for more seamless service.
<b><o:p></o:p></b></p>
<p class="MsoNormal">The shelter Reservations feature in HIFIS also allows planning stay days, weeks or months ahead. Perhaps your shelter has access to beds on-site that are for people who need help with detox. A client can be receiving treatment at a detox
centre and be in need of a bed at a certain time, whether it is on the same day or in a month’s time.<o:p></o:p></p>
<p class="MsoNormal">For returning clients, making a reservation when client leave for the day can help the shelter plan ahead for the night and allow for the client to set aside their belongings securely. In HIFIS, service providers can easily record clients’
personal items through the Storage module, if this service is offered at their location.
