<p class="MsoNormal"><a href="https://www.canada.ca/fr/emploi-developpement-social/programmes/sans-abri.html"><span lang="FR-CA">Vers un chez-soi : la stratégie canadienne de lutte contre l’itinérance</span></a><span lang="FR-CA"> aide les communautés, comme
la </span><a href="https://www.dufferincounty.ca/"><span lang="FR-CA">Corporation du comté de Dufferin</span></a><span lang="FR-CA"> en Ontario, à réduire l’itinérance au moyen d’une approche coordonnée et axée sur les données. L’accès coordonné est également
une priorité du programme Vers un chez-soi. Grâce à l’accès coordonné, les communautés s’efforcent de rationaliser leurs systèmes de prestation de services et de créer une plus grande diversité quant au soutien qui est offert aux personnes en situation d’itinérance
ou à risque de le devenir pour régler leur problème de logement.</span><span lang="FR-CA" style="line-height:105%"><o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Le comté de Dufferin en Ontario est une communauté de Vers un chez-soi qui a mis en œuvre avec succès l’accès coordonné. Au cours de la dernière année et demie, cette communauté a réduit de plus de 26 % le nombre de personnes
en situation d’itinérance chronique.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">« C’est tout simplement la bonne chose à faire », affirme Anna McGregor, directrice des services communautaires. « Personne ne devrait se retrouver sans chez-soi. Il en coûte bien plus de garder une personne dans le réseau
de lutte contre l’itinérance que de l’en faire sortir. »<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Cet engagement a encouragé la communauté à utiliser des données de qualité et en temps réel tirées de sa liste nominative pour essayer de nouvelles façons de collaborer plus étroitement afin d’atteindre des objectifs
communs. Avant d’établir sa liste en temps réel des personnes en situation d’itinérance (parfois appelée liste d’identificateurs uniques), le comté de Dufferin ne faisait pas le suivi de sa population de personnes en situation d’itinérance. Malgré deux dénombrements
ponctuels, il ne connaissait pas le nombre réel de personnes en situation d’itinérance, de leur emplacement ou de leurs besoins.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">L’équipe organise maintenant des rencontres hebdomadaires avec des partenaires de services au moyen d’une table de concertation sur l’accès coordonné. Grâce à ces concertations hebdomadaires, les membres de l’équipe élaborent
des plans en logement pour les personnes en situation d’itinérance et mettent en commun leurs ressources pour pouvoir répondre aux besoins de sa population de personnes en situation d’itinérance chronique. En utilisant le pouvoir que confèrent les données
en temps réel, ils repèrent les lacunes et cherchent des solutions.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">« Nous pouvons constater que nous contribuons à changer les choses », a déclaré Anna. « Le suivi que nous faisons chaque semaine à la table de concertation de l’accès coordonné nous permet de voir à ce que les gens ne
tombent pas dans les failles du système. La tenue à jour de la liste s’est révélée fort précieuse. »<o:p></o:p></span></p>

(CCN). </span><span lang="FR-CA"><o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">L’objectif</span><span lang="FR-CA" style="color:#040000"> des PBC est d’honorer les bénévoles et de mettre de l’avant la contribution que des personnes, des organismes sans but lucratif, des entreprises à vocation sociale
et commerciale apportent à leur communauté sous forme de bénévolat. Les Prix visent à inciter les Canadiens de toutes les couches de la société à découvrir de nouvelles façons de changer les choses dans leur communauté.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Pour<span style="color:#040000"> sélectionner les lauréats des PBC, le gouvernement du Canada s’en remet aux conseils et aux recommandations du CCN, qui</span><span style="color:#333333">
</span><span style="color:#040000">est composé de chefs de file de la </span>collectivité<span style="color:#040000"> et d’agents de changement du secteur du bénévolat qui sont passionnés et engagés dans le domaine du bénévolat. Les membres du CCN conseillent

<p class="MsoNormal">Dufferin County in Ontario is a Reaching Home community that has successfully implemented Coordinated Access. Over the last year and a half, they have reduced the number of people experiencing chronic homelessness by over 26%.<o:p></o:p></p>
<p class="MsoNormal">“This is simply the right thing to do,” says Anna McGregor, Director of Community Services. “No one should have no place to call home. It costs far more to keep people in the homelessness system than to get them out of it.”<o:p></o:p></p>
<p class="MsoNormal">That commitment has inspired the community to use quality, real-time data from their By-Name List to try new ways of working closer together to achieve common goals. Before they had a real-time list of people experiencing homelessness (also
sometimes called a Unique Identifier List), Dufferin did not track its homeless population as accurately. They had two Point-in-Time counts, but between those counts they did not accurately know how many people were experiencing homelessness, where they were,
or what their needs were.<o:p></o:p></p>
<p class="MsoNormal">The team now hosts weekly service partners through a Coordinated Access Table. Through these weekly case conferences, they develop housing plans for people experiencing homelessness, and pool their resources to make sure the needs of its
chronic homeless population are being met. Using the power of real-time data, they identify gaps and work on solutions.<o:p></o:p></p>
<p class="MsoNormal">“You get to see that the work is making a difference,” Anna said. “The constant follow-up each week at the Coordinated Access Table ensures that people don’t slip through ‘system’ cracks. Keeping the List up to date has been invaluable
for that.”<o:p></o:p></p>
<p class="MsoNormal">“We definitely feel we are part of a big, strong movement to effect real change,” she said.<o:p></o:p></p>

research and knowledge mobilization specific to youth homelessness prevention and housing stabilization. Through its work, MtS has gathered learnings and developed tools to support the prevention of youth homelessness in Canada.

<p class="MsoNormal">The Homeless Individuals and Family Information System (HIFIS) Release 4.0.59.2 (59.2) will soon be available for download. This new HIFIS release includes Point-in-Time (PiT) Count mandatory updates in preparation for the launch of Everyone
Counts that will take place in March and April 2021. <span style="line-height:105%">

<p class="MsoNormal"><span style="color:#040000;mso-fareast-language:EN-CA">The objective of CVA is to recognize volunteers and bring national prominence to the contributions individuals, not-for-profit organizations, social enterprises and businesses make
to their communities through volunteering. The goal of the awards is to inspire Canadians from all walks of life to find new ways of making a difference in their communities.<o:p></o:p></span></p>
<p class="MsoNormal">To<span style="color:#040000;mso-fareast-language:EN-CA"> select CVA recipients, the Government of Canada relies on the advice and recommendations of the NAC, which</span><span style="color:#333333;mso-fareast-language:EN-CA">
