aide les communautés, comme la </span><a href="https://www.dufferincounty.ca/"><span lang="FR-CA" style="line-height:106%">Corporation du comté de Dufferin</span></a><span lang="FR-CA" style="line-height:106%">, à réduire l’itinérance au moyen d’une approche
coordonnée et axée sur les données. L’accès coordonné est également une priorité du programme Vers un chez-soi. Grâce à l’accès coordonné, les communautés s’efforcent de rationaliser leurs systèmes de prestation de services et de créer une plus grande diversité
quant au soutien qui est offert aux personnes en situation d’itinérance ou à risque de le devenir pour régler leur problème de logement.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:106%">Le comté de Dufferin en Ontario est une communauté de Vers un chez-soi qui a mis en œuvre avec succès l’accès coordonné. Au cours des deux dernières années, cette communauté a réduit de 50 % le
nombre de personnes en situation d’itinérance chronique.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:106%">Afin d’atteindre cette diminution, le Comté de Dufferin s’est concentré sur la mise en œuvre conjointe de services de soutien et d’allocations de logement. Il a également fait preuve d’une souplesse
accrue en ce qui concerne le mode de prestation des services pendant la pandémie de COVID-19.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:106%">En tant que communauté de
</span><a href="https://fr.bfzcanada.ca/"><span lang="FR-CA" style="line-height:106%">Prêt pour zéro Canada</span></a><span lang="FR-CA" style="line-height:106%">, le Comté de Dufferin utilise les données issues de sa liste nominative, une liste en temps réel
de personnes en situation d’itinérance au sein de la communauté, pour poursuivre leur objectif de mettre fin à l’itinérance chronique.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:106%">Anna McGregor, directrice des Services communautaires du Comté de Dufferin, remarque que « la liste nominative fournit les données indiquant si les changements apportés fonctionnent ou non. La
liste a également permis de constater que l’accès coordonné fonctionne en offrant un logement à plus de personnes et en ayant un plus grand nombre de partenaires collaboratifs. »

<p class="MsoNormal"><a href="https://www.canada.ca/en/employment-social-development/programs/homelessness.html"><span style="line-height:106%">Reaching Home: Canada’s Homelessness Strategy</span></a><span style="line-height:106%"> supports communities, like
the </span><a href="https://www.dufferincounty.ca/"><span style="line-height:106%">Corporation of the County of Dufferin</span></a><span style="line-height:106%">, to drive reductions in homelessness using a coordinated, data-driven approach.
<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">Dufferin County in Ontario is a Reaching Home community that has successfully implemented Coordinated Access. Coordinated Access allows for communities to streamline their service delivery systems and create
greater efficiencies in how people experiencing or at-risk of homelessness are helped with their housing challenges.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">Over the last two years, Dufferin County has now reduced the number of people experiencing chronic homelessness by 50%.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">To achieve this reduction, Dufferin County focused on the combined implementation of support services and housing allowances and also became more flexible regarding how services were delivered during the during
the COVID-19 pandemic. <o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">As a </span><a href="https://bfzcanada.ca/"><span style="line-height:106%">Built for Zero Canada</span></a><span style="line-height:106%"> community, Dufferin County uses data from their By-Name List (BNL),
a real-time list of people experiencing homelessness in their community, to work towards their goal of ending chronic homelessness.
<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">“Our BNL provides the numbers to show whether or not the changes we made work. It has shown that our Coordinated Access works by getting more people housing and having more partners working collectively,”
says Anna McGregor, Director, Community Services, Dufferin County Community Services.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:106%">To continue building on this success, the Community Entity will continue to provide additional training for members of its Coordinated Access Table, to help strengthen the collective leadership roles they
have and to make the overall group stronger. It also plans on realigning funding into housing allowances and support services, when possible.

<p class="MsoNormal"><span lang="EN-US" style="line-height:106%">Part of the program's role is to deepen Solutions Lab participants’ understanding of specific housing challenges by exposing them to the perspectives of other participants. By enabling groups
to work together, the Labs can develop collective, relevant and responsive solutions to these challenges. The successful solutions will support the strengthening of the affordable housing sector across Canada.<o:p></o:p></span></p>
