<p class="MsoNormal">Under <a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/index-eng.html">
Reaching Home: Canada’s Homelessness Strategy</a>, communities receiving funding under the
<a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/funding-financement/designated-comms-designees-eng.html">
Designated Communities stream</a> must implement a Coordinated Access system and adopt an outcomes-based approach, including the implementation of a unique identifier list. A unique identifier list, also known as a
<a href="https://bfzcanada.ca/by-name-lists/">By-Name List</a>, makes it possible for a community to measure, share and monitor outcomes at the local-level.<o:p></o:p></p>
<p class="MsoNormal">The Community Entity in Brantford, Ontario, the <a href="https://www.brantford.ca/en/index.aspx">
City of Brantford</a>, in partnership with <a href="https://caeh.ca/">the Canadian Alliance to End Homelessness</a> and as part of
<a href="https://bfzcanada.ca/by-name-lists/">Built for Zero Canada</a><span class="MsoHyperlink"><span style="text-decoration:none">
</span></span><span class="MsoHyperlink"><span style="color:windowtext;text-decoration:none">for Ontario communities, was able to achieve a Quality By-Name List (QNBL) and achieve</span></span><span class="MsoHyperlink"><span style="color:windowtext">
</span></span><span class="MsoHyperlink"><span style="color:windowtext;text-decoration:none">the Reaching Home level of
</span></span><a href="https://www.canada.ca/en/employment-social-development/programs/homelessness/directives.html#h2.3-h3.5">Coordinated Access</a>
<span class="MsoHyperlink"><span style="color:windowtext;text-decoration:none">within a one-year period</span></span>. While the community previously used a different database for their homelessness system of care, implementing the Homeless Individuals and
Families Information System (HIFIS) has allowed Brantford-Brant to enhance collaborative efforts and streamline real-time data collection with service partners in their work to end chronic homelessness.
<o:p></o:p></p>
<p class="MsoNormal"><span style="color:black;background:white">“Since we’ve implemented our system, the biggest piece has been the improved collaboration between partners. With the ability to collect and share information, we’re seeing greater partnership
to deliver care. We have improved data quality, and our processes are even more streamlined. We don’t have the delays we used to when we relied on paper forms because staff are entering information in real-time,” explained Hanah Itner, Project Coordinator
at the City of Brantford. </span><o:p></o:p></p>
<p class="MsoNormal">Having developed key strategies such as working towards shared goals, adopting clear policies and procedures, and maintaining effective communication, Brantford-Brant was able to implement a Coordinated Access system and build trust with
their community partners while also taking the next step towards ending chronic homelessness.<o:p></o:p></p>

<p class="MsoNormal">The 2021-22 Community Homelessness Report (CHR) reporting cycle<span style="line-height:105%"> will be launched in the coming weeks by the Homelessness Policy Directorate (HPD) at Infrastructure Canada</span>.<span style="line-height:105%">
This second report will cover activities and include data spanning the third year of the program (April 1, 2021 through March 31, 2022).<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="EN-US" style="line-height:105%">As identified in the</span><span lang="EN-US">
</span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/directives-eng.html"><span lang="EN-US" style="line-height:105%">Reaching Home Directives</span></a><span lang="EN-US" style="line-height:105%">, communities receiving funding from the
Designated Communities stream </span>(outside of Quebec) <span lang="EN-US" style="line-height:105%">
are required to submit a CHR each year. This requirement also applies to communities funded through the Territorial Homelessness stream that also receive funding to implement Coordinated Access.

<p class="MsoNormal"><span lang="EN-US" style="line-height:105%">Through their CHR, communities self-assess their progress with Reaching Home implementation, which includes the following key components:<o:p></o:p></span></p>

</span><span lang="EN-US" style="line-height:115%">Unique Identifier List (also known as a By-Name List or, simply, a List).<o:p></o:p></span></li></ul>
<p class="MsoNormal"><span lang="EN-US" style="line-height:105%">The CHR is designed to support local discussions and decision making, with a focus on the full picture of homelessness at the community level and the collective efforts of service providers working
across the homeless-serving system to address it. Communities are encouraged to use their CHR data to develop clear plans of action that help them to reach their homelessness reduction targets.
</span><o:p></o:p></p>
<p class="MsoNormal"><span style="line-height:105%">To support the launch of the 2021-22 CHR reporting cycle, an updated
</span>CHR reporting template and <span style="line-height:105%">a new "CHR: 2021-22 Reporting Cycle Overview" document will be released in the coming weeks. The Overview document will highlight key changes made to the reporting template, as well as provide
an overview of the CHR, Coordinated Access and the Outcomes-based approach</span>. Both the reporting template and the Overview document<span style="line-height:105%">
</span>will be made available for download on the Homelessness Learning Hub's <a href="https://www.homelessnesslearninghub.ca/library/resources/reaching-home-community-homelessness-report-reporting-tools">
Community Homelessness Report: Reporting Tools</a> e-course. <b>Community Entities (CEs) will be notified by email once these tools are available online.</b><o:p></o:p></p>
<p class="MsoNormal">A number of additional resources will be released to further support CEs in completing their 2021-22 report, including a section-by-section CHR Reference Guide, an updated Questions and Answers document, recorded training webcasts and additional
training opportunities (e.g., invitations to Questions and Answers sessions led by the Homelessness Policy Directorate).

<p class="MsoNormal">The completed and Community Advisory Board (CAB) approved 2021-22 CHR is due to Service Canada by
<b><span style="line-height:105%">October 7, 2022</span></b>.<o:p></o:p></p>

<p class="MsoNormal">Through the Urban, Rural and Northern (URN) Indigenous Housing Working Group, the
<a href="https://www.placetocallhome.ca/national-housing-council">National Housing Council</a> released a
<a href="https://assets.cmhc-schl.gc.ca/sites/place-to-call-home/pdfs/report-recommendations-minister-housing-diversity-inclusion-urban-rural-nih-en.pdf">
new report and recommendations to the Minister of Housing and Diversity and Inclusion on Urban, Rural, and Northern Indigenous Housing.</a> This report was commissioned by the National Housing Council to provide the Minister of Housing and Diversity and Inclusion
with recommendations on how to address existing gaps in housing approaches and outcomes in urban, rural, and northern Indigenous housing.<o:p></o:p></p>
<p class="MsoNormal">The final report contains four recommendations to the Minister, including:

<p class="MsoNormal">The paper, released by the <a href="https://www.homelesshub.ca/about-us">
Canadian Observatory on Homelessness</a>, notes that for people experiencing homelessness, COVID-19 was much more of a crisis than it was for others. Many street-based services were closing down and a large umber of people around the world were living in shelters
with a very high risk of infection transmission. <o:p></o:p></p>
<p class="MsoNormal">The paper explores how the crisis was managed for this population in practice, what lessons can be learned and what can be done in the future should we face a similar sanitary crisis. While the paper focuses on the United Kingdom nations,
case studies and analysis from North America, Europe and Asia are also included.

<p class="MsoNormal">An interview conducted by the <a href="https://www.homelesshub.ca/about-us">
Canadian Observatory on Homelessness</a> shared some highlights on what it would take to shift towards prevention of youth homelessness based on the analysis of a pilot project conducted in Toronto, Ontario.
<o:p></o:p></p>
<p class="MsoNormal">In Toronto, the COVID-19 pandemic placed an already-strained homelessness shelter system under greater pressure. With shelters forced to decrease their capacity by 35%, a group of youth-focused service providers worked with the City of
Toronto to move 70 young people experiencing homelessness into a downtown hotel.
<o:p></o:p></p>
<p class="MsoNormal"><a href="https://makingtheshiftinc.ca/">Making the Shift</a> Principal Investigators Dr. Amanda Noble and Dr. Naomi Thulien investigated how young people in downtown Toronto were faring in the pandemic over 2021 and listened to the experiences
of youth using downtown shelters and youth participating in the hotel pilot. Their research project is titled
<a href="http://makingtheshiftinc.ca/projects/">“Responding to youth homelessness in the midst of a pandemic: Shifting to collaborative, prevention-based services in a large urban setting”</a>, with the final report set to be completed soon.

mettre en œuvre un système d’Accès coordonné et adopter une approche axée sur les résultats, y compris la mise en place d’une liste d’identificateurs uniques. Une telle liste, également connue sous le nom de

et dans le cadre de l’initiative </span><a href="https://fr.bfzcanada.ca/"><span lang="FR-CA">Prêt pour zéro Canada</span></a><span class="MsoHyperlink"><span lang="FR-CA" style="color:windowtext;text-decoration:none"> pour les communautés ontariennes, est
parvenue à créer une liste par nom de qualité et à atteindre le niveau Vers un chez-soi de l’</span></span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/directives-fra.html#h2.3-h3.4"><span lang="FR-CA">accès coordonné</span></a>
<span class="MsoHyperlink"><span lang="FR-CA" style="color:windowtext;text-decoration:none">au bout d’un an</span></span><span lang="FR-CA">. Même si la communauté utilisait une base de données différente pour son système de soins pour les personnes en situation
d’itinérance, la mise en œuvre du Système d’information sur les personnes et les familles sans abri (SISA) a permis à Brantford-Brant d’améliorer les efforts de collaboration et de rationaliser la collecte de données en temps réel avec les partenaires de service
dans leur travail pour mettre fin à l’itinérance chronique. </span><span lang="FR-CA"><o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="color:black;background:white">« Depuis que nous avons mis en place notre système, la collaboration s’est considérablement accrue entre les partenaires. Grâce à la possibilité de recueillir et d’échanger des renseignements,
nous constatons un plus grand partenariat pour assurer la prestation des soins. Nous avons amélioré la qualité des données, et nos processus sont encore plus simplifiés. Nous n’avons plus à faire face à des retards comme lorsque nous utilisions des formulaires
papier, car le personnel saisit les renseignements en temps réel », explique Hanah Itner, coordinatrice de projet à la ville de Brantford.

<p class="MsoNormal"><span lang="FR-CA">Le cycle de production de rapport 2021-2022 du Rapport communautaire en matière d’itinérance (RCMI) sera lancé dans les prochaines semaines par la Direction des politiques en matière d’itinérance (DPMI) d’Infrastructure
Canada. Ce deuxième rapport portera sur les activités et fera état des données qui s’étalent sur la troisième année du programme (du 1<sup>er</sup> avril 2021 au 31 mars 2022).<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Tel qu’indiqué dans </span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/directives-fra.html"><span lang="FR-CA">les Directives de Vers un chez-soi</span></a><span lang="FR-CA">, les communautés qui
reçoivent du financement par l’entremise du volet Communautés désignées (à l’extérieur du Québec) sont tenues de produire un RCMI chaque année. Cette exigence s’applique également aux communautés financées par l’entremise du volet Itinérance dans les territoires
qui reçoivent également du financement pour mettre en œuvre l’Accès coordonné.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Par l’entremise de leur RCMI, les communautés auto-évaluent leurs progrès dans la mise en œuvre de Vers un chez-soi, ce qui comprend les éléments clés suivants :<o:p></o:p></span></p>

<p class="MsoNormal"><span lang="FR-CA">Le RCMI est conçue pour appuyer les discussions et la prise de décisions au niveau local, en mettant l’accent sur la situation globale de l’itinérance au niveau communautaire et les efforts collectifs des fournisseurs
de services qui travaillent dans l’ensemble du système de services en matière d’itinérance. Les communautés sont encouragées à utiliser les données de leur RCMI pour élaborer des plans d’action clairs qui les aideront à atteindre leurs objectifs de réduction
de l’itinérance.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Pour appuyer le lancement du cycle de production de rapports des RCMI de 2021-2022, un gabarit du RCMI mis à jour et un nouveau document, « Aperçu du cycle de production de rapports du RCMI de 2021-2022 », seront partagés
au cours des prochaines semaines. L’Aperçu soulignera les principaux changements apportés au gabarit, ainsi qu’un aperçu du RCMI, de l’Accès coordonné et de l’approche axée sur les résultats. Le gabarit et l’Aperçu pourront être téléchargés sur le cours en

<p class="MsoNormal"><span lang="FR-CA">Plusieurs ressources supplémentaires seront partagées pour aider les EC davantage à compléter leur rapport 2021-2022, y compris un guide de référence du RCMI qui fournit de l’information pour chacune des sections à compléter,
un document fournissant des questions et réponses mises à jour, des webinaires de formation enregistrés et des possibilités de formation supplémentaires (p. ex. des invitations à des séances de questions et réponses menées par la Direction des politiques en

qu’il faudrait faire pour prévenir l’itinérance chez les jeunes, selon l’analyse d’un projet pilote mené à Toronto, en Ontario. À Toronto, le système de refuges pour les personnes en situation d’itinérance déjà mis à rude épreuve a été grandement éprouvé par
la pandémie de la COVID-19. Les refuges ayant été contraints de réduire leur capacité d’accueil de 35 %, un groupe de fournisseurs de services axés sur la jeunesse a collaboré avec la ville de Toronto pour loger 70 jeunes en situation d’itinérance dans un
hôtel du centre-ville. </span><span lang="FR-CA"><o:p></o:p></span></p>
