Spotlight: Sydney - Cape Breton (Nova Scotia) Uses Reaching Home Funding to Provide Support to Their Community<o:p></o:p></span></span></span></p>

<p class="MsoNormal"><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/index-eng.html"><span style="line-height:105%">Reaching Home: Canada’s Homelessness Strategy</span></a><span style="line-height:105%"> is a community-based program aimed at
preventing and reducing homelessness that provides funding and support to urban, Indigenous, territorial and rural and remote communities across Canada. The
</span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/funding-financement/designated-comms-designees-eng.html"><span style="line-height:105%">Designated Communities stream</span></a><span style="line-height:105%"> provides long-term stable
funding to 64 urban communities outside the territories that face significant issues with homelessness. Organizations within the Designated Community can apply for Reaching Home funding through their Community Entity to support priorities identified in their
community’s homelessness plan.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:105%">The Community Entity in the Designated Community of Sydney - Cape Breton, Nova Scotia, the
</span><a href="https://www.endhomelessnesstoday.ca/"><span style="line-height:105%">Cardeil Place Society</span></a><span class="MsoHyperlink"><span style="line-height:105%">,</span></span><span style="line-height:105%"> has partnered with community organizations
to direct Reaching Home funding to the community to help address challenges posed to the homeless-serving sector throughout the COVID-19 pandemic.

<p class="MsoNormal"><span style="line-height:105%">This includes the </span><a href="https://www.allycentreofcapebreton.com/"><span style="line-height:105%">Ally Centre</span></a><span class="MsoHyperlink"><span style="line-height:105%">, a non-profit organization
whose purpose is to prevent the spread of blood borne pathogens in the community and to create a supportive environment for those infected and/or affected by blood borne pathogens</span></span><span style="line-height:105%">. Reaching Home funding allowed
the Ally Centre to expand the hours of their Comfort Centre, providing a safe space for individuals in need or experiencing homelessness. In one case, Comfort Centre staff were able to perform a life-saving medical intervention to one of their clients who
was in distress. These actions may not have been possible had the Care Centre not been operating during the expanded hours.
<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:105%">The Ally Centre also manages a
</span><a href="https://www.allycentreofcapebreton.com/services"><span style="line-height:105%">Mobile Outreach Street Health (MOSH) bus</span></a><span style="line-height:105%"> that provides health services to vulnerable people who are located in the communities
outside of Sydney – Cape Breton, N.S. Reaching Home funding supports the office for the Registered Nurse who works on the MOSH bus and also covers the costs associated with operating the bus. The MOSH bus allows for health services to be provided in a proactive
way by going where the need is, making it easier for people who might be nervous accessing services in the traditional way. The mobile clinic also travels with peer-support workers who have lived-experience, providing a first hand understanding of the struggles
many people face when trying to access primary care. <o:p></o:p></span></p>

<p class="MsoNormal"><span style="line-height:105%">The CHR is a Reaching Home reporting tool for communities funded through the Designated Communities stream and the three territorial capitals funded through the Territorial Homelessness stream. The CHR helps
communities to self-assess their progress with meeting minimum requirements under Reaching Home, including:

<p class="MsoNormal"><span style="line-height:105%">The first CHR reporting period under Reaching Home covered the first two fiscal years of the program, from April 1, 2019 through March 31, 2021. As such, CHR results presented in this article are current as
of March 31, 2021<a style="mso-footnote-id:ftn1" href="#_ftn1" name="_ftnref1" title=""><span class="MsoFootnoteReference"><span class="MsoFootnoteReference"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif;mso-fareast-language:EN-US">[1]</span></span></span></a>
and are based on a total of 59 communities.<o:p></o:p></span></p>
<p class="MsoNormal" style="margin-top:12.0pt"><b><span style="line-height:105%">Coordinated Access Implementation: Results</span></b><b> by Core Component</b><b><span style="line-height:105%"><o:p></o:p></span></b></p>
<p class="MsoNormal"><span lang="EN-US">Under Reaching Home, Coordinated Access is defined as<b>
</b>a way for communities to bring consistency to the process by which people experiencing or at-risk of homelessness access housing and related services within a geographic area. In the CHR, communities self-assess their progress with meeting 25 Coordinated
Access minimum requirements, which are outlined in the </span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/directives-eng.html"><span lang="EN-US">Reaching Home Directives.</span></a><b><span lang="EN-US"><o:p></o:p></span></b></p>
<p class="MsoNormal">The implementation of Reaching Home represents a significant shift in the way that homelessness is addressed. Making progress requires buy-in and engagement with many people across a community, every step of the way. Overall, the first
set of CHRs showed that communities were at different stages with Coordinated Access implementation, which was expected.
<o:p></o:p></p>
<p class="MsoNormal">Across all communities, on average, 51% of the minimum requirements were met for Coordinated Access within the first two years of the program (by March 2021). This was accomplished during a very challenging period, where communities also
had to manage the impacts of COVID-19. That being said, communities that were making good progress before COVID-19<b>
</b>shared through their CHRs that they were better able to respond to the pandemic because of it. Many communities gave examples of how COVID-19 compelled various actors to work more closely together, such as those from the housing, health and correctional
sectors. <o:p></o:p></p>
<p class="MsoNormal">Rates of progress varied by each core component for Coordinated Access, as illustrated in the graph below. Across the country, average percentages of requirements met were:

<p class="MsoNormal">Several of the core components have requirements for written documentation, such as the requirement to have a prioritization policy as part of the vacancy matching and referral process. Across the country, on average, communities met 41
percent of these requirements (see Policy Development in the graph below).<o:p></o:p></p>

<p class="MsoNormal">Communities have continued to make progress in the third year of the program, which is being documented through the second set of CHRs (covering April 2021 through March 2022). We know that many communities are also keeping track of their
progress with Reaching Home implementation using their Canadian Alliance to End Homelessness scorecards, given the overlap in questions between these tools and the CHR.<o:p></o:p></p>
<p class="MsoNormal">Results from the second CHR reporting cycle are being analyzed over the coming months. Stay tuned for future articles about results of program implementation up to March 31, 2022.<o:p></o:p></p>

<p class="MsoNormal"><span style="line-height:105%">In September 2022, the </span>
<a href="https://www.statcan.gc.ca/en/subjects-start/crime_and_justice"><span style="line-height:105%">Canadian Centre for Justice and Community Safety Statistics</span></a><span style="line-height:105%"> released a report entitled
</span><a href="https://www150.statcan.gc.ca/n1/daily-quotidien/220916/dq220916c-eng.htm"><span style="line-height:105%">“Indigenous shelters for victims of abuse in Canada, 2020/2021”</span></a><span style="line-height:105%">. The report examines the characteristics
of Indigenous shelters in Canada, such as the number of annual admissions and the types of services provided to the clients they serve.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:105%">In 2020/2021, there were 93 shelters operating across Canada with ties to First Nations, Métis or Inuit communities or organizations that were primarily mandated to serve victims of abuse. These shelters,
which mainly serve people from Indigenous communities, play an important role for victims of abuse by offering them a safe environment and basic living needs, as well as various other types of support and outreach services. The report notes that just over
half (54%) of all Indigenous shelters were in rural areas and about 4 in 10 (39%) were on reserves. Data also demonstrated that 80% of Indigenous shelters were in First Nations, Métis or Inuit communities, while the remaining 20% had other ties to Indigenous
communities or organizations, such as being owned or operated by an Indigenous governing body.
<o:p></o:p></span></p>
<p class="MsoNormal"><span style="line-height:105%">The report also noted that the pandemic affected the operations of shelters and their ability to provide support and services to victims of abuse. About one-third of Indigenous (32%) and non-Indigenous (34%)
shelters reported that, overall, the pandemic has affected their ability to provide services to a great extent.

of the </span><a href="https://www23.statcan.gc.ca/imdb/p2SV.pl?Function=getSurvey&SDDS=3328"><span style="line-height:105%">Survey of Residential Facilities for Victims of Abuse</span></a><span style="line-height:105%"> and presents information on Indigenous
shelters in Canada. The article contains information on how these shelters adapted their operations during the first year of the pandemic with the implementation of different measures and practices. In addition, it presents detailed information on the characteristics
of Indigenous shelters for victims of abuse, as well as on the residents who are staying in these facilities.

Canada offer Shelter Diversion (SD) and Eviction Prevention (EP) programs and learn more about their success. This project ultimately aims to build the capacity of communities to implement SD and EP programs and is being funded through Reaching Home’s Community

<p class="MsoNormal"><span style="line-height:105%">Shelter Diversion focuses on helping people who show up at emergency shelters by identifying immediate alternate temporary housing arrangements in their communities, and if necessary, connecting people with
services and financial assistance to help them return to permanent housing. <o:p>

<p class="MsoNormal"><span style="line-height:105%">Eviction Prevention is a form of both early intervention and housing stabilization for people at-risk of becoming or returning to homelessness. In practice, EP refers to a range of strategies and program interventions
designed to keep individuals and families at (imminent) risk of eviction in their homes with tenure and thereby help them avoid entering into homelessness.

<span lang="FR-CA">est un programme communautaire visant à prévenir et à réduire l’itinérance. Il soutient financièrement des communautés urbaines, autochtones, territoriales, rurales et éloignées du Canada. Le
</span></span><a href="https://www.infrastructure.gc.ca/homelessness-sans-abri/funding-financement/designated-comms-designees-fra.html"><span lang="FR-CA" style="line-height:105%">volet Communautés désignées</span></a><span lang="FR-CA" style="line-height:105%">
fournit un financement stable à long terme à 64 communautés urbaines à l’extérieur des territoires qui font face à d’importants problèmes en matière d’itinérance. Les organismes d’une communauté désignée peuvent demander un financement de Vers un chez-soi
par l’intermédiaire de leur Entité communautaire afin d’appuyer les priorités figurant dans leur plan communautaire de lutte contre l’itinérance.</span><span style="line-height:105%"><o:p></o:p></span></p>

l’Entité communautaire dans la communauté désignée de Sydney – Cap-Breton, en Nouvelle-Écosse, s’est associée à des organismes communautaires pour diriger le financement de Vers un chez-soi au sein de la communauté afin d’aider le secteur des services aux
personnes en situation d’itinérance à relever les défis que pose la pandémie de COVID-19.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:105%">L’organisme sans but lucratif
</span><a href="https://www.allycentreofcapebreton.com/"><span lang="FR-CA" style="line-height:105%">Ally Centre</span></a><span class="MsoHyperlink"><span lang="FR-CA" style="line-height:105%"> fait partie de ce partenariat (en anglais seulement)</span></span><span lang="FR-CA" style="line-height:105%">.
<span class="MsoHyperlink">Cet organisme a pour but de prévenir la propagation des agents pathogènes à diffusion hématogène dans la communauté et de créer un environnement de soutien pour les personnes infectées ou affectées par ces agents pathogènes</span>.
Grâce au financement de Vers un chez-soi, Ally Centre a pu prolonger les heures d’ouverture de son centre de réconfort, qui offre un lieu sécuritaire aux personnes dans le besoin ou en situation d’itinérance. Le personnel de ce centre a notamment pu pratiquer
une intervention médicale qui a sauvé la vie de l’un de ses clients en détresse. Il n’aurait peut-être pas été possible de poser ce geste si le centre n’avait pas été ouvert pendant les heures prolongées.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:105%">Ally Centre gère aussi un
</span><a href="https://www.allycentreofcapebreton.com/services"><span lang="FR-CA" style="line-height:105%">autobus offrant un service mobile de sensibilisation à la santé dans la rue</span></a><span style="line-height:105%">
<span class="MsoHyperlink"><span lang="FR-CA">(en anglais seulement)</span></span><span lang="FR-CA"> qui fournit des services de santé aux personnes vulnérables qui se trouvent dans des communautés à l’extérieur de Sydney – Cap-Breton, en Nouvelle-Écosse.
Le programme Vers un chez-soi soutient financièrement le bureau de l’infirmière autorisée qui travaille à bord de l’autobus et couvre les coûts associés au fonctionnement de l’autobus. L’autobus permet d’offrir des services de santé de façon proactive en
se rendant là où les besoins se font sentir, ce qui facilite l’accès de services de santé aux personnes qui pourraient être nerveuses d’obtenir ces services de la manière traditionnelle. La clinique mobile se déplace aussi avec des intervenants de soutien
par les pairs qui ont une expérience vécue, et qui comprennent donc les difficultés auxquelles de nombreuses personnes se heurtent lorsqu’elles tentent d’avoir accès à des soins primaires.<o:p></o:p></span></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:105%">NB : Afin de permettre une réponse rapide et efficace dans les communautés devant l’éclosion de la COVID-19, le programme Vers un chez soi a instauré des flexibilités temporaires au titre de ses
directives, dont l’admissibilité des services médicaux et de santé. Les services médicaux et de santé sont une catégorie d’activités et de dépenses admissibles temporaire, qui sera retirée en date du 31 mars 2023. Pour en savoir plus sur le programme Vers

<p class="MsoNormal"><span lang="FR-CA">Le présent article est le deuxième d’une série d’articles mettant en lumière les principaux résultats des Rapports communautaires en matière d’itinérance (RCMI) 2019-2021.

<p class="MsoNormal"><span lang="FR-CA">Le RCMI est un outil de production de rapports du programme Vers un chez-soi destiné aux communautés qui reçoivent du financement du volet Communautés désignées et aux trois capitales territoriales financées par l’intermédiaire
du volet Itinérance dans les territoires. Le RCMI aide les communautés à autoévaluer leurs progrès par rapport à la satisfaction des exigences minimales de Vers un chez-soi, notamment :

<p class="MsoNormal"><span lang="FR-CA">Le premier cycle de production du RCMI de Vers un chez-soi a couvert les deux premiers exercices financiers du programme, soit la période du 1<sup>er</sup> avril 2019 au 31 mars 2021. Par conséquent, les résultats du
RCMI présentés dans le présent article sont à jour en date du 31 mars 2021<a style="mso-footnote-id:ftn2" href="#_ftn2" name="_ftnref2" title=""><span class="MsoFootnoteReference"><span class="MsoFootnoteReference"><span lang="FR-CA" style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif;mso-fareast-language:EN-US">[2]</span></span></span></a>
et sont établis en fonction d’un total de 59 communautés. <o:p></o:p></span></p>
<p class="MsoNormal" style="margin-top:12.0pt"><b><span lang="FR-CA">Mise en œuvre de l’accès coordonné : Résultats par composante clé</span></b><b><span lang="FR-CA" style="line-height:105%"><o:p></o:p></span></b></p>
<p class="MsoNormal"><span lang="FR-CA">Dans le cadre de Vers un chez-soi, un système d’accès coordonné est défini comme un moyen pour les communautés d’assurer l’uniformité du processus par lequel les personnes en situation d’itinérance ou à risque de le devenir
ont accès à un logement et à des services connexes dans une région géographique donnée. Dans le RCMI, les communautés évaluent elles-mêmes leurs progrès par rapport à la satisfaction des 25 exigences minimales relatives à l’accès coordonné, qui sont décrites
dans les directives de Vers un chez-soi.<b><o:p></o:p></b></span></p>
<p class="MsoNormal"><span lang="FR-CA">La mise en œuvre de Vers un chez-soi représente un changement considérable dans la façon dont la question de l’itinérance est abordée. Pour réaliser des progrès, il faut obtenir l’adhésion et la participation de nombreuses
personnes au sein d’une communauté, à chaque étape du processus. Dans l’ensemble, la première série du RCMI a révélé que les communautés avaient atteint des étapes différentes dans la mise en œuvre de l’accès coordonné, ce qui était prévisible.
<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Dans l’ensemble des communautés, en moyenne, 51 % des exigences minimales relatives à l’accès coordonné ont été satisfaites au cours des deux premières années du programme (jusqu’à mars 2021). Ce résultat a été atteint
au cours d’une période très difficile, où les communautés devaient également gérer les répercussions de la pandémie de la COVID-19. Cela dit, les communautés qui progressaient bien avant la pandémie ont indiqué dans leurs RCMI que les travaux qu’elles avaient
réalisés leur avaient permis de mieux répondre à la crise</span><span lang="FR-CA"> sanitaire. De nombreuses communautés ont donné des exemples de la façon dont la COVID-19 a encouragé divers acteurs à travailler plus étroitement ensemble, notamment ceux des
secteurs du logement, de la santé et des services correctionnels. </span><span lang="FR-CA"><o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA">Les taux de progression ont varié selon chaque composante clé de l’accès coordonné, comme le montre le graphique ci-dessous. Dans l’ensemble du pays, le pourcentage moyen d’exigences satisfaites était de :

<p class="MsoNormal"><span lang="FR-CA">Plusieurs des composantes clés nécessitent des documents écrits, par exemple l’exigence de disposer d’une politique de priorisation dans le cadre du processus de jumelage et d’aiguillage vers les
</span><span lang="FR-CA">logements vacants. Dans l’ensemble du pays, en moyenne, 41 % de ces exigences ont été satisfaites par les communautés (voir la section Élaboration des politiques dans le graphique ci-dessous).<o:p></o:p></span></p>

communautés assurent également le suivi de leurs progrès par rapport à la mise en œuvre de Vers un chez soi à l’aide des fiches d’évaluation de l’Alliance canadienne pour mettre fin à l’itinérance, étant donné le chevauchement des questions entre cet outil

publié le rapport </span><a href="https://www150.statcan.gc.ca/n1/daily-quotidien/220916/dq220916c-fra.htm"><i><span lang="FR-CA" style="line-height:105%">Maisons d’hébergement autochtones pour les victimes de violence au Canada, 2020-2021</span></i></a><span lang="FR-CA" style="line-height:105%">.
Le rapport examine les caractéristiques des maisons d’hébergement autochtones au Canada, comme le nombre d’admissions annuelles et les types de services fournis aux clients qu’ils servent.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:105%">En 2020-2021, le Canada comptait 93 maisons d’hébergement en activité rattachées à des communautés ou à des organismes des Premières Nations, des Métis ou des Inuits, dont le mandat principal
était de fournir des services aux victimes de violence. Ces maisons d’hébergement, qui servent principalement les personnes des communautés autochtones, jouent un rôle important auprès des victimes de violence en leur offrant un environnement sécuritaire et
en répondant à leurs besoins essentiels, en plus d’assurer différents types de services de soutien et de services dans la communauté. Dans le rapport, on indique qu’un peu plus de la moitié (54 %) des maisons d’hébergement autochtones étaient situées en milieu
rural et qu’environ 4 établissements sur 10 (39 %) se trouvaient dans des réserves. Selon les données, 80 % des maisons d’hébergement autochtones étaient situées dans des communautés des Premières Nations, des Métis ou des Inuits, tandis que la proportion
restante de 20 % avait d’autres liens avec des communautés ou des organismes autochtones, comme le fait d’appartenir à une instance dirigeante autochtone ou d’être exploité par cette dernière.<o:p></o:p></span></p>
<p class="MsoNormal"><span lang="FR-CA" style="line-height:105%">Toujours selon le rapport, la pandémie a affecté les activités des maisons d’hébergement et leur capacité à fournir du soutien et des services aux victimes de violence. Environ le tiers des maisons
d’hébergement autochtones (32 %) et des maisons d’hébergement non autochtones (34 %) ont déclaré que, dans l’ensemble, la pandémie a eu une grande incidence sur leur capacité à fournir des services.<o:p></o:p></span></p>

mène une enquête pour recenser le nombre d’organismes au Canada qui offrent des programmes de prévention des expulsions (PE) et de détournement des refuges (DR) et pour en apprendre plus sur leur réussite. Ce projet vise au bout du compte à renforcer la capacité
