who has now successfully secured long-term, suitable housing after diligently working toward her goals and progressing through all three tiers of a housing program. Grateful for the support she received, she has chosen to volunteer weekly at Elizabeth Fry,
allowing the organization to continue witnessing her growth while offering assistance with her daily living needs as she maintains her own residence.<o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk154124247"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif;mso-ligatures:none">When she first came to Elizabeth Fry, she was experiencing
homelessness and hitchhiking, deciding that Thunder Bay was where she wanted to stay. Initially, she was placed at a shelter for women fleeing violence, but after being discharged from their program, she reached out to Elizabeth Fry again for a bed. During
her time spent with Elizabeth Fry she thrived, consistently attending programs and seeking external employment, all while focusing on her housing goals. This client was also working hard toward her recovery goals and attending countless AA meetings and secured
herself with a sponsor that would assist her with maintaining her sobriety. The staff at Elizabeth Fry are excited to see her continue moving forward with the plans she has set in motion as she embarks on the next phase of her journey.<o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk154124247"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif;mso-ligatures:none">Elizabeth Fry has seen considerable success among women
and gender-diverse individuals transitioning into private market housing. Many of the clients are independently working toward their goals, with staff offering guidance as needed. Taking control of their lives, they attend appointments, seek additional financial
support for housing, participate in rental viewings, and engage in counseling services. Clients are encouraged to take responsibility for their needs, with help from Elizabeth Fry to prepare them for their eventual transition from a transitional space to independent
living. Elizabeth Fry’s role is to motivate and support them in achieving success as individuals within the community, providing essential resources along the way. The top priority is ensuring clients have access to stable housing. Once this basic need is
met, improvements are seen in client’s health, self-management, and life skills.<o:p></o:p></span></span></p>

<i>Canadian Methodology for Calculating Homeless-Serving System Inflow, Outflow and Active Homelessness Data</i>. Using this methodology will ensure that communities using HIFIS are measuring homelessness and tracking changes over time in the same way, increasing
data consistency for the homeless-serving sector as a whole. <o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk154124247"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">The COR was developed in partnership with the Canadian Alliance to End
Homelessness (CAEH) and ACRE Consulting. To create the COR, elements of two HIFIS reports were consolidated – the Reaching Home Community Outcomes Report (maintained by HICC) and the CAEH Inflow/Outflow Report (maintained by CAEH). Once the COR is available,
these previous reports will be retired. <o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk154124247"><b><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">With the COR, communities will have access to a single, more comprehensive
tool that measures homelessness at the systems level using best practices in the homeless-serving sector.
</span></b></span><span style="mso-bookmark:_Hlk154124247"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">The COR includes aggregate data for all inflow and outflow pathways, giving communities a comprehensive picture of homelessness.
It also includes an option to display person-specific (identifiable) data, revealing who is currently experiencing homelessness, as well as who inflowed into homelessness, who outflowed from homelessness, and who transitioned into chronic homelessness. This
feature allows communities to use the COR to measure outcomes at the individual level, which can inform service planning activities. Person-specific data can also be used to support data quality audits, helping to confirm who is included or excluded from aggregate-level
results. <o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk154124247"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">Communities will be required to use the COR for all reporting to HICC by
March 31, 2026, including the Reaching Home Community Homelessness Report and the Homelessness Indicator Project (HIP). Communities will also be encouraged to use the COR for their data submissions to CAEH’s Built for Zero Canada (BFZ-C). BFZ-C Improvement
Advisors will be reaching out to communities to further explore and support this opportunity.<o:p></o:p></span></span></p>

peut accueillir jusqu’à 17 personnes à la fois, notamment avec deux lits associés à une table d’accès coordonné au logement pour les clients en situation de crise ou d’itinérance. L’objectif est d’apporter un soutien bienveillant, en allant à la rencontre
des clients là où ils se trouvent et en travaillant avec eux dans le cadre de la gestion de cas afin de les aider à réussir dans des domaines tels que le logement, la sécurité alimentaire, les besoins fondamentaux, l’emploi, les services de consultation, la

est arrivée à la Société Elizabeth Fry, elle était en situation d’itinérance et faisait de l’auto-stop, avant de décider que Thunder Bay était l’endroit où elle voulait rester. Dans un premier temps, elle a été placée au refuge pour les femmes fuyant la violence,
mais lors de sa sortie de ce programme, elle a de nouveau contacté Elizabeth Fry pour obtenir un lit. Pendant son séjour à la Société Elizabeth Fry, elle s’est épanouie, participant régulièrement à des programmes et cherchant un emploi à l’extérieur, tout
en se concentrant sur ses objectifs en matière de logement. Cette cliente travaillait également dur pour atteindre ses objectifs de rétablissement, assistait à d’innombrables réunions des AA et avait trouvé un parrain qui l’aiderait à maintenir la sobriété.
Le personnel de la Société Elizabeth Fry se réjouit de la voir continuer à aller de l’avant avec les projets qu’elle a mis en place, alors qu’elle entame la prochaine étape de son parcours.<o:p></o:p></span></span></span></p>

objectifs, avec le soutien du personnel si nécessaire. Prenant le contrôle de leur vie, ils se rendent à leurs rendez-vous, recherchent un soutien financier supplémentaire pour le logement, participent à des visites de logements et profitent de services de
consultation. Les clients sont encouragés à prendre en charge leurs besoins, avec l’aide de la Société Elizabeth Fry pour les préparer à leur éventuelle transition d’un espace transitoire à un mode de vie indépendant. Le rôle de la Société Elizabeth Fry consiste
à les motiver et à les aider à réussir en tant qu’individus au sein de la communauté, en leur fournissant des ressources essentielles tout au long de leur parcours. La priorité absolue est de veiller à ce que les clientes aient accès à un logement stable.
Une fois ce besoin fondamental satisfait, des améliorations sont constatées au niveau de la santé, de l’autogestion et des aptitudes à la vie quotidienne du client.<o:p></o:p></span></span></span></p>

Le RRC comprend des données agrégées pour toutes les voies d'entrée et de sortie, ce qui donne aux communautés une image complète de l’itinérance. Il comprend également une option permettant d'afficher des données propres à la personne (identifiables), révélant
qui est actuellement en situation d’itinérance, qui est entré en situation d’itinérance, qui est sorti d’une situation d’itinérance et qui est passé à une situation d’itinérance chronique. Cette caractéristique permet aux communautés d'utiliser le RRC pour
mesurer les résultats au niveau individuel, ce qui peut éclairer les activités de planification des services. Les données propres à la personne peuvent également être utilisées pour soutenir les vérifications de qualité des données, en aidant à confirmer qui
est inclus ou exclus des résultats agrégés. <o:p></o:p></span></span></span></p>

encouragées à utiliser le RRC pour leurs soumissions de données à Prêt pour zéro Canada (PPZ-C) de l'ACMFI. Les conseillers en amélioration de PPZ-C communiqueront avec les communautés afin d'explorer et de soutenir davantage cette possibilité.<o:p></o:p></span></span></span></p>
