<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">Hamilton’s health sector and homeless-serving organizations collaborate regularly to tackle health-related challenges
among people experiencing homelessness. This includes participation of health partners in key system level meetings, involvement of health sector programs in Coordinated Access, and working together to build capacity of the homeless-serving sector on health
topics. For example, together they have organized health-related training opportunities for frontline staff of their homeless-serving sector to support learning in areas of key identified gaps such as harm reduction and mental health de-escalation. In 2025,
these sessions reached 130 frontline staff across agencies participating in Coordinated Access with a high level of satisfaction among participants. Joint health and housing team planning is underway for additional sessions and strategies for 2026.

<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">Hamilton has experienced a significant increase in refugee claimants and asylum seekers arriving in the region,
many of whom lack completed settlement paperwork. Historically, this group has accounted for 25–35% of local shelter capacity, creating challenges for service providers who are unfamiliar with settlement processes.<o:p></o:p></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">By leveraging Coordinated Access supports already in place across the community, the

Assistance Program</span></a>.<span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif"> In partnership with community agencies, the City of Hamilton developed a comprehensive suite of case management supports and shelter beds for asylum
seekers and refugees who access mainstream emergency shelters and drop-ins to address these gaps. These programs were able to achieve 90 day shelter stays on average with permanent housing outcomes for individuals, relieving significant pressures to the system
overall.<o:p></o:p></span></p>

<i>BC and Federal Coordinated Access System and the Homeless Individuals and Families Information System (HIFIS) Expansion Alignment Project</i> was launched, referred to as the </span><a href="https://www.bchousing.org/projects-partners/housing-provider-info/the-alignment-project"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif;color:#467886">Alignment
Project</span></a><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">. The goal is to bring together different programs and services that help people experiencing or at risk of homelessness, so they work better together. This includes
aligning roles, responsibilities and systems (like HIFIS). The shared aim for all project partners is to make homelessness rare, brief, and one-time.<o:p></o:p></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">Through this project, partners are building on what’s already working in BC and integrating successful approaches

with helping communities across Canada to optimize their response to homelessness, adopt proven practices and drive continuous improvement.<o:p></o:p></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">The project is starting in
<b>10 priority communities</b>: Abbotsford, Chilliwack, Cowichan Valley, Kamloops, Kelowna, Nanaimo, Nelson, Penticton, Prince George, and Greater Victoria. Communities will be supported to fully implement Reaching Home through these joint efforts, given that
Coordinated Access, HIFIS and Outcomes-Based Approach minimum requirements are now fully aligned with the provincial Belonging in BC strategy. Each of the priority communities receive Reaching Home Designated Communities funding except Penticton, which is
testing the approach as a <br>
non-Reaching Home community.<o:p></o:p></span></p>

oversee planning, implementation, reporting, and funding alignment. Efforts to further support an integrated approach to local governance will follow, supported by these new teams and tables.<o:p></o:p></span></li><li class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%;mso-list:l3 level1 lfo3">
<span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">In BC, the provincial instance of HIFIS is managed by BC Housing as the System Administrator. A new consent form has been launched to support the onboarding of Reaching Home Community
Entities to their new Community Administrator role, as well as new HIFIS users that will participate in the local Coordinated Access system.<o:p></o:p></span></li></ul>

<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span lang="FR-CA" style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">Le secteur de la santé et les organismes de services en itinérance de Hamilton collaborent régulièrement
pour relever les défis liés à la santé chez les personnes en situation d'itinérance. Cela comprend la participation de partenaires du secteur de la santé à des réunions clés au niveau du système, l'implication des programmes du secteur de la santé dans l'accès
coordonné et la collaboration pour renforcer les capacités du secteur d'aide aux personnes en situation d’itinérance sur les questions de santé. Par exemple, ils ont organisé ensemble des formations sur la santé à l'intention du personnel de première ligne
qui travaille auprès des personnes en situation d’itinérance afin de combler les lacunes identifiées dans des domaines clés tels que la réduction des risques et la désescalade en matière de santé mentale. En 2025, ces séances ont été livrées à 130 membres
du personnel de première ligne de toutes les agences participant dans l'accès coordonné, avec un niveau de satisfaction élevé parmi les participants. Une équipe conjointe chargée de la santé et du logement planifie actuellement des séances et des stratégies
supplémentaires pour 2026.<o:p></o:p></span></p>

de réfugié dans la région, dont beaucoup n'ont pas rempli tous les documents nécessaires à l’établissement. Historiquement, ce groupe représentait 25 à 35 % de la capacité d'accueil des refuges locaux, ce qui posait des difficultés aux fournisseurs de services
qui n’étaient pas familiarisés avec les processus d’établissement.<o:p></o:p></span></p>

En partenariat avec des organismes communautaires, la ville de Hamilton a mis en place un ensemble complet de mesures d'aide à la gestion de cas et de lits d'hébergement pour les demandeurs d'asile et les réfugiés ayant recours aux refuges d'urgence et aux
centres d'accueil traditionnels, afin de combler ces lacunes. En moyenne, ces programmes ont permis de réaliser des séjours de 90 jours en refuge et des solutions de logement permanentes pour les personnes concernées, ce qui a considérablement allégé la pression
sur l'ensemble du système.<o:p></o:p></span></p>

<b>10 communautés prioritaires</b> <b>:</b> Abbotsford, Chilliwack, Cowichan Valley, Kamloops, Kelowna, Nanaimo, Nelson, Penticton, Prince George et Grand Victoria. Ces communautés bénéficieront d'un soutien pour mettre pleinement en œuvre le programme Vers
un chez-soi grâce à ces efforts conjoints, et ce, compte tenu que les exigences minimales en matière d'accès coordonné, du SISA et de l'approche axée sur les résultats sont désormais pleinement conformes à la stratégie provinciale « Belonging in BC ». Chacune
des communautés prioritaires reçoit un financement dans le cadre du volet de financement « Communautés désignées » du programme Vers un chez-soi, à l'exception de Penticton qui met actuellement à l’essai l'approche en tant que communauté hors de la portée
de Vers un chez-soi..<o:p></o:p></span></p>

additionnels seront déployés afin de consolider une approche intégrée de la gouvernance locale, avec l’appui de ces nouvelles équipes et tables.<o:p></o:p></span></p>
