chronic homelessness in the community. <o:p></o:p></span></span></p>

that provides wraparound support services to people who have been experiencing homelessness. Services include rental assistance and housing navigation to help individuals secure stable housing and navigate the housing market. Basic needs support offers other
essentials, like bedding, household supplies and transportation during an individual’s transition into housing. SOC also conducts advocacy and outreach activities and works with local organizations and landlords to ensure sustainable housing solutions.<o:p></o:p></span></span></p>
<p class="MsoNormal" style="margin-bottom:8.0pt;line-height:105%"><span style="mso-bookmark:_Hlk222123611"><span style="font-size:12.0pt;line-height:105%;font-family:"Arial",sans-serif">With a philosophy that embraces the value of therapeutic relationships,
SOC staff deliver client-centered case management services which strive to support the unique needs of each participant. The program is designed to present a clear pathway from emergency housing to independent living, with ongoing support to ensure long-term
success and reduce homelessness in Moose Jaw.<o:p></o:p></span></span></p>

places at-risk tenants into supportive services to reverse the conditions causing their imminent eviction.<o:p></o:p></span></span></p>

an inventory of warm-up and cool-down locations, along with emergency shelter support services available within the city of Moose Jaw. This resource includes contact information and guidance for concerned citizens on whom to contact when encountering a person
who is unhoused or experiencing housing insecurity and is at risk due to weather conditions. The organization’s strong sense of community and collaborative spirit have helped create a powerful continuum of support in the city and for the community members
experiencing or at risk of homelessness. <o:p></o:p></span></span></p>

(en anglais seulement) qui offre des <a name="_Hlk214949887">mesures de soutien globales</a> qui ont été en situation d’itinérance. Ces services comprennent l’aide au loyer et la navigation des options de logement afin d’aider les personnes à obtenir un logement
stable et à naviguer dans le marché de l’habitation. Parmi les services offerts, certains sont axés sur les besoins fondamentaux qui visent à fournir d’autres produits essentiels, comme la literie, les fournitures ménagères et le transport pendant la transition
d’une personne vers un logement. La SOC mène également des activités en matière de proximité et de mobilisation, et collabore avec des organisations locales et des propriétaires afin de garantir des solutions de logement durables.<o:p></o:p></span></p>

visant à répondre aux besoins uniques de chaque participant. Le programme est conçu pour tracer une voie d’accès facile entre l’hébergement d’urgence et la vie autonome, avec un accompagnement continu pour assurer le succès à long terme et réduire l’itinérance
à Moose Jaw.<o:p></o:p></span></p>

d’urgence offerts dans la ville de Moose Jaw. Cette ressource comprend des coordonnées et des directives à l’intention des citoyens préoccupés, indiquant qui contacter lorsqu’ils rencontrent une personne sans logement ou en situation de précarité résidentielle
et exposée à des risques en raison des conditions météorologiques. Le fort esprit communautaire et la collaboration étroite de l’organisme ont contribué à créer un continuum de soutien solide dans la ville, au bénéfice des personnes en situation d’itinérance

comment le renforcement des exigences du programme Vers un chez-soi peut favoriser l’uniformisation des pratiques et soutenir la rétention des connaissances. La séance comprenait une table ronde nationale animée par LICC, à laquelle participaient des représentants

ont également discuté des outils favorisant l’amélioration continue, tels que les systèmes de formation standardisés, la coordination de la documentation et le partage transparent des données.
