E-DRIVE [SHAPE SURVEY FOR B.C. RESIDENTS]

SHAPE, Safety and Health in Arts Production and Entertainment, a non-profit organization promoting health and safety in B.C.'s Motion Picture and Performing Arts industry invites Equity members resident in B.C. to participate in a short health and safety survey. 
 
Grand Prize: $400 cash
Second Prize: Two Chicago Blackhawks vs. Vancouver Canucks tickets 
for November 22, 2009 @ 7pm. (Value $347).
Both prizes to be drawn Friday, November 6th, 2009
Early Bird Prize: $100 cash
To be drawn Friday, October 16th 2009
