If you are filling out the Activities of Daily Living Index, 

BEWARE! This is a Tricky form.

To those in the health field, activities of daily living means self care. That is NOT what it means on this form. On this form, ADL means not only self care but also ability to function in the workplace and in the community. In the small print at the top, you will see that explained, but the title can still lead one to miss the full implications, and not everyone reads the small print And while some questions are clear, others are not. 

Let's illustrate how that plays out. 

Take item 6: Exhibits normal limits of functioning with respect to impulse control and behaviour:.

The person may be able to carry out self care without any problem in this regard. That would be class 1. However, could the person exhibit such limits in the workplace? That may well be Class 4. Correct answer in that case would be 4.

The Adjudicator is looking especially for 3's and 4's. 

Should you have completed the form without this understanding of the situation, you should follow up with a letter of explanation, that you were using the health care definition of ADL.

On another matter, the form fails to ask the essential questions. However, you may answer them in any case on page 13.

Does the person have substantial difficulty with self care?

Does the person have substantial difficulty functioning in the workplace?

Does the person have substantial difficulty functioning in the community?

If the answer to any of these is yes, will this problem be likely to persist for at least a year?
