[homeles_ot-l] Vet Clinic Information (Ottawa Mission)

The next vet clinic to be held at the Ottawa Mission will be Saturday, November 26th, 2- 6 PM. We will send out referral forms on Monday, October 17th, 2011. Referrals will not be accepted prior to this. Please see below for details regarding eligibility and referral process.

 

 

ELIGIBILITY:

 

                Clients MUST:

·         Have an existing case worker at a partnering agency (*see below)

OR

Make regular use of shelter/outreach services

·         Have a high-risk/street involved cat or dog

·         Have a cat or dog in need of shots

 

 

Clients MUST NOT:

·         Have an existing veterinarian-client relationship

·         Have a regular income (other than OW/ODSP)

 

*Clients must be referred to the clinic by a case worker OTHER THAN OW/ODSP workers (Referring agencies include CMHA, ACTT, Community Health Centres, Options Bytown, YSB, Community Laundry Co-op, etc...)  The case worker can be a nurse, social worker, housing worker, etc. as long as he/she knows the client. Clients should not seek out a case worker for the sole purpose of getting a referral.  If clients do not have a case worker, they should contact vet clinics in Ottawa to request a sliding scale.

 

REFERRAL PROCESS FOR CASE WORKERS:

 

1.	Case worker at referring agency to assess client eligibility for the program
2.	One month prior to vet clinic:

◦       Complete referral form (maximum 3 pets/client)

◦       Fax form to Ottawa Mission: 613-234-7588

3.	Appointments will be booked on a first come, first served basis by the Ottawa Mission two weeks before the Clinic

◦       Appointments for clients without a phone or contact number will be made through case worker

PLEASE NOTE:

The next clinic to be held at The Ottawa Mission will be on November 26, 2011.  Referrals will be sent out on October 17th and accepted only after this time. Clients will be contacted mid-November to schedule appointments.  The vet clinic provides preventative health care; please advise clients with sick pets not to wait until the clinic to have them treated.
