Call for nominations: Eliana Roman Memorial Award

Criteria:  

The Eliana Roman Memorial Award was established in 2010 by the Dream
Team to recognize organizations or individuals who have made outstanding
contributions to the advancement of Human Rights for people who live
with mental health and addictions.  Recipients of the award have through
their daily practice demonstrated a commitment to working for positive
changes in the lives of people who have experienced homeless and who
live with mental health and addiction issues. 

Who can nominate:   

*	Individuals or organizations may submit a nomination letter to
the selection committee by 5:00pm, March 31, 2011
*	The letter should be addressed to Dream Team Award Selections
Committee, 1678 Bloor Street West, Toronto, ON M6P 1A9
*	The nomination letter should include 

	*	Name and contact information of the nominee 
	*	A brief biography of the nominee(up to 350 words)
	*	A brief statement highlighting achievements,  community
activities and the reasons why a particular individual should be
recognized(up to 500 words)
	*	Name and contact information of the nominating
individual or organization

Recognition: 

The winner of the Eliana Roman Memorial Award will be presented at the
Dream Team annual fundraising dinner on April 28th 2011.  About 350
people including, consumer survivor advocates, tenants and professionals
of the mental health and housing sector attend this event every year.
