[homeles_ot-l] Seeking nominations for Mental Health Service Awards / Certificats de mérite en santé mentale by Oct. 21

The Canadian Mental Health Association, Ottawa Branch, is seeking nominations for our annual Mental Health Service Awards. The awards recognize contributions in one or more of the following categories:

*       Professional or volunteer frontline provider of services to consumers of mental health services
*       Concept and/or delivery of a program of extraordinary effectiveness, timeliness and/or innovation
*       People with a lived experience of mental health issues / Consumers of mental health services
*       Family/friends of people with lived experience
*       Volunteer participation on Boards and Committees Involved in the conception, formulation, and realization of policy/programs.

Preference will be given to nominees whose contributions have not previously been recognized and who have involved in their service for several years.

We would appreciate it if you could circulate the Awards application and brochure to your networks.

The deadline for nominations is October 21st .The Awards will be presented at a celebration on November 25th at noon. Nominations should be mailed or faxed to the coordinates below.
