Dear Community Partners,

 

The Intensive Case Management Program, a partnership between the Oasis Program of the Sandy Hill CHC and CMHA-Ottawa, is no longer accepting referrals.  

 

The program's mandate is to provide intensive case management and stable housing through rent supplements to 96 people.  We currently have 68 clients enrolled into the program.  We have over 80 completed referrals, yet we can only accept 28 more clients into the program.  

 

We will be selecting the 28 clients based on their complexity of needs (the highest needs being accepted first).  The decision is based on the information contained in the referral package and the face-to-face interview.  The deciding committee is composed of the Intensive Case Management Team, the Housing Coordinator from CMHA and a community partner with expertise in homeless health.  

 

We will keep our community partners involved in the future as caseload space becomes available.
