The ICM Program is currently full.  We do have spots that open up
periodically.  The ICM Program offers intensive case management to clients
living with very complex needs:

 

Substance use disorder, 

Chronic homelessness,

Long term history of hospitalizations,

Repeated involvement with the criminal justice system.

 

To refer your client and to ensure a proper assessment of needs, please
complete the Eligibility Referral Form and contact Nancy Rouselle (Systems
Navigator, CMHA-Ottawa Branch).  Please call her at 613-737-7791 ext. 198
