Hello and good afternoon,
I’m Chris from Operation Come Home and I wanted to inquire with you about referring youth to our new Peer Case Management Program: R.E.A.C.H. (Radical, Empathetic, Acceptance, Connections, Harm Reduction)
We are specializing in helping youth (ages 16-29) manage and or reduce their substance use. We’re doing this by meeting them in the community where they are at and finding goals that they would like to work towards in order to get to a place where they’d like to be. By helping with their goals, we are hoping to help with their substance use and in addition, their mental health. For things we cannot provide we will be referring them to agencies in the community that can give the proper care that they deserve. By being in the community we can not just refer someone to a resource but go along with them to these agencies and follow through with them and the service provider.
