URGENT MEMBER ADVISORY: MEDICAL INSURANCE RECEIPT CALCULATION ERROR

Equity Members have recently received tax receipts for medical insurance
premiums for the 2013 tax year.  We have now discovered that there is an
error in the calculation and Equity will be issuing new medical
insurance receipts this week. PLEASE DO NOT USE YOUR CURRENT MEDICAL
INSURANCE RECEIPT WHEN FILING YOUR TAXES. Dues Receipts are not affected
by this error, and the Insurance Week Eligibility statement on your
medical insurance receipt is also correct.

Equity's new insurance plan took effect on April 1st, 2013.
Unfortunately, the incorporation of this change led to a miscalculation
of the portion of the premium eligible as a medical expense.

To resolve this issue, receipts reflecting the correct amount will be
issued and mailed by Friday March 21, 2014. The corrected receipt will
be marked as "AMENDED". Please destroy the old receipt.

We have re-programmed our systems to ensure this will not happen again.

We are embarrassed by our waste of postage and any confusion the
incorrect receipt may have caused you. Please accept our sincere
apology.
