Hi,
 
Please find attached the Referral Package for the Intensive Case Management Program.  A partnership between the Sandy Hill Community Health Centre and CMHA Ottawa.
 
The Intensive Case Management Program works with complex clients who meet the following criteria:
 
* Substance use disorder,
* Chronic homelessness,
* Complex physical or mental health needs,
* Long history of interactions with the criminal justice system,
* Numerous interactions with the emergency health system.
 
The Intensive Case Management Program is a long-term case management service that adheres to the housing first model.  The program is not a housing provider, we work with our clients to help them find suitable housing.
 
Please feel free to print out and complete the included referral packages.  Please complete the forms with care to ensure a successful referral process.
