[homeles_ot-l] Daybreak Vacancies - New Application Process

I would like to inform our community partners that Daybreak is currently
looking to fill some female vacancies in our homes. Daybreak is a Non-Profit
housing organization that provides Rent Geared to Income housing to single
adults (male and female) who require minimal supports to live independently
and have the appropriate community supports for their needs (i.e. addictions
counseling, social worker, psychiatrist, etc.).  Our homes are drug and
alcohol free homes and tenants must be able to perform their activities of
daily living independently (bathing, laundry, cooking, cleaning, etc.).
Daybreak operates 2 homes for women and 3 homes for men. All the houses fit
in well with their surrounding neighbourhoods, and the homes are very much a
part of the local community. Daybreak residents have their own private,
furnished rooms, and live in a community environment, sharing common areas.


 

If you or your coworkers know of a client who you feel is well suited to our
housing, please have them complete an application with the Housing Registry.


 

Please note that Daybreak has recently changed our application process and
no longer accepts the old 'Daybreak' or 'Ottawa Supportive Housing'
applications. Our new process requires all applicants to submit their
Application for Subsidized Housing in Ottawa to The Registry.  Below are
further instructions on how to make an application to live at Daybreak
Housing.

 

What Is the Process to Apply to Live at Daybreak?

1.  Fill out the Social Housing Registry Application Form .  Be sure to
complete all required fields.  You may obtain application forms from The
Registry office, the Registry's webpage (www.housingregistry.ca) or from the
Daybreak office.

2.  Remember to check off the Daybreak box in section 9. (Supportive Housing
section) of the Registry application, along with any other providers you
wish to apply to.

3.  Please note that Daybreak only offers rooms.  To be placed on Daybreak's
waitlist an applicant must indicated, in section 5. (Housing Preferences),
that they are willing to reside in a room (in a rooming house).  If an
applicant answers 'no' to this question but selects Daybreak in section 9,
they will not be placed on Daybreak's waiting list as Daybreak only offers
rooms. 

4.  Submit your application and all supporting documents to the Registry
either by mail or in person. PLEASE NOTE THAT THE REGISTRY WILL NOT ACCEPT
APPLICATIONS BY FAX.

5.  After the application is received by The Registry, you will be placed on
a waiting list.

6.  When an application nears review for a vacancy, Daybreak staff will
contact the applicant to confirm their interest in housing with Daybreak and
book an interview to further review eligibility/suitability to our Daybreak
homes.

7.  Once you have met with Daybreak staff for an Intake Interview, staff
will contact applicant references to confirm eligibility/suitability to
Daybreak housing and applicants will then be contacted to be informed of the
resulting decision.

8.  If any contact information changes, please contact The Registry right
away to note these changes, so we can continue to get in touch with
applicants as necessary.
