Your Health: Hospital case managers act as agents for patients in and out of hospital

Once a patient is discharged from the hospital, the last thing he or she wants is to be readmitted. Unfortunately for some that is exactly what happens. Re-admissions occur for many reasons, but the major causes cited within research include uncertainty about medications or simply not taking medications as prescribed; failure to follow-up with the primary care physician within 7-10 days after discharge; and lack of understanding about the disease process.
