[CANUFNET] Commemorative or Memorial Tree planting program survey

Good morning,

The following survey is an initiative of the City of Ottawa and is for the purposes of gathering information related to Commemorative or Memorial Tree planting programs across Canada. Individual responses will be kept confidential however, group results may be share with other municipalities.

The “Client” refers to the person(s) requesting the tree planting.

This survey will take approximately 5-10 minutes to complete.

The deadline to complete the survey is September 1, 2015.

Bonjour,

Ce sondage a été conçu par la Ville d’Ottawa pour recueillir des données sur les programmes de plantation d’arbres commémoratifs mis en place au Canada. Toutes vos réponses demeureront confidentielles, mais les résultats collectifs pourraient être transmis à d’autres municipalités.
Le terme « client » désigne la personne qui demande la plantation d’un arbre.
Ce sondage devrait prendre de 5 à 10 minutes à remplir.
La date limite pour compléter ce sondage est de 1 Septembre 2015.
